How to Enter and More Information
- Review the Official Entry Form and Rules & Regulations from the MS-HUG Healthcare Innovation Awards website at www.mshug.org/awards. All entries must be submitted on-line.
- Once completed, BOTH healthcare organization and technology solution partner must sign the completed Release Form (last page of entry document) and fax to 425-645-2020. Please note: Your submission will not be considered unless the Release Form is faxed in.
- Submission deadline is December 3rd, 2007 at 5:00 PM Pacific Standard Time. After this date, all entries and content will be considered final.
